Geisinger Health Plan may not cover services deemed experimental or investigational, cosmetic procedures not medically necessary, routine foot care except for diabetes or peripheral vascular disease, and elective abortions. Custodial care, personal comfort items, and over-the-counter medications typically are not covered. Fertility treatments like in vitro fertilization, reproductive procedures, or reversal of sterilization may not be covered. Coverage exclusions might also apply to weight loss surgery if not deemed medically necessary or treatments outside the U.S. without prior authorization.
